How was the Moon formed?
The moon was formed due to a giant impact of a mars sized body hitting the newly formed Proto-earth, this blasted material in orbit of the earth, including the moon

Around 4.5 billion years

Why does the Moon only show one side to us all of the time?
So now the Moon makes one rotation every 29.5 days. That happens to also be the time it takes for the Moon to complete one revolution around the Earth. Which ,means only one side is able to be seen.

about 3.8 centimeters per year

Eventually the Moons increasing distance will cause the Earth's rotation to slow and tides will become weaker due to the Moons weakening gravitational pull.
